Role of myo-inositol in the treatment of acne associated with polycystic ovary syndrome

Acne is a disorder of the pilosebaceous unit. It is characterized by a highly complex etiopathology that includes inflammation, hyperkeratinization, increased sebum production, colonization by Cutibacterium acnes, hyperandrogenemia, and hyperinsulinemia. This condition—along with hirsutism, androgenetic alopecia, and acanthosis nigricans—is a highly prevalent cutaneous manifestation of polycystic ovary syndrome (PCOS).

Although conventional therapies are effective treatment options, they are not free from side effects, which can reduce patient compliance. In this context, particular attention has been focused on nutraceutical supplements—a diverse group of molecules with significant potential for reducing inflammation, hyperkeratinization, hyperseborrhea, and hyperinsulinemia. Myo-inositol has proven effective in improving several signs and symptoms in patients with polycystic ovaries, including a reduction in body mass index (BMI), free testosterone levels, and dehydroepiandrosterone sulfate (DHEAS) levels, as well as an improvement in ovarian function and insulin sensitivity. The authors conducted a retrospective study (https://doi.org/10.3390/ph18020251) involving 200 patients with PCOS. Over a 6-month period, the study analyzed the effects of supplementation with LEVIGON™—a specific nutraceutical formulation containing myo-inositol, microlipodispersed® magnesium, and folic acid—on the clinical presentation of acne and hirsutism. Supplementation with LEVIGON™ led to a significant reduction in BMI, total testosterone, free testosterone, and DHEAS levels, thereby improving the clinical manifestations of acne and hirsutism. Furthermore, the impact of acne on quality of life—assessed using the Cardiff Acne Disability Index (CADI) and the Dermatology Life Quality Index (DLQI)—showed significant improvement after 3 and 6 months. Women with hirsutism also experienced a significant improvement in their Ferriman-Gallwey score after 3 and 6 months (p < 0.0001; p < 0.0001, respectively, compared to baseline). Supplementation with myo-inositol, combined with Microlipodispersed® magnesium, proved highly effective in reducing acne and hirsutism in patients with polycystic ovaries. Furthermore, no side effects were observed, confirming excellent patient compliance.
